A dementia diagnosis can be overwhelming – and it may feel like everything has changed. But our information and advice can help – whether you want to better understand dementia symptoms or need support finding and arranging social care.

Our support can be thought of as a Holistic MOT - a simple way to check the key areas that can help someone live well with dementia.

1) Finances
Worries about money can affect every part of life. We can help you understand what support you may be entitled to and assist with practical steps, including Benefit Checks and help with relevant forms (e.g., Attendance Allowance, Pension Credit and other entitlements).

2) Health & wellbeing
We can talk through what’s worrying you, help you think about staying safe and well, and connect you to appropriate local support. Where needed, we can also refer/signpost to organisations that support with aids, adaptations and wider wellbeing needs.

3) Home
Home matters, especially when living with dementia. We can help you think about whether the home is safe, warm and suitable as needs change, and we can refer into our Home Energy Checks service where relevant.

4) Social connections
Loneliness and isolation can increase stress for both the person with dementia and those supporting them. We can help people reconnect with community and social group options where appropriate.

5) Future planning
Many people want to talk through “what next?” - care and support at home, navigating social care, powers of attorney, and planning ahead. We can provide tailored guidance and help people feel more in control.

Support for carers and family
Carers often come to us because they’re juggling multiple pressures - supporting a loved one while also trying to manage finances, housing, forms, services and their own wellbeing. We can:

- help carers understand available support and entitlements
- help families navigate practical decisions and next steps
- connect carers to wellbeing opportunities

Age Cymru Powys is not a clinical dementia diagnosis service. Our role is to provide practical advice, guidance, and support, and to help you connect with specialist dementia services where needed.
