The Project Management Team works across all Social Care to deliver bespoke housing, accommodation and support models.The Team leads on many projects and works closely with key stakeholders, citizens and their representatives and others to achieve and deliver high quality services and outcomes.
We manage a specialist housing waiting list and develop accommodation support models working in partnership with professionals, registered social landlords and partner agencies.We are lead managing agent responsible for all supported living properties and claim Additional Variable Service Charge to maintain the communal areas in multi-occupancy properties/services.
The Team develop and co-ordinate Person Centred Planning (PCP) approaches / initiatives working with the Standards of Wrexham Services (SWS) Group.The SWS Group identified the need for a unsupported friendship group where people can meet up and attend different activities, they were supported to create the Facebook page.
