SUPPORT AND ADVICE SERVICE FOR ANYONE WHO IS 55 AND OVER

The aim of the service is to assist older people to be as independent as possible in the home of their choice.

Support is provided to older people in any tenure or they can be in hospital or in a nursing home etc. Assistance and support can be as long as is required - one or two visits or until a particular need has been met. The service at present is free to anyone 55 and over.

These are just a few examples of support and assistance that can be provided::

· Arranged meals on wheels, home cleaning service

· Arranged installation of Lifeline 400 - this is an alarm system that the older person can call in an emergency should they fall or not be feeling well

· Arranged adaptations such as walk-in-shower or grab rails

· Arranged for a re-assessment of pathway so that service user can have access for a wheelchair into their home

· Completed housing register forms for rehousing, housing benefit, council tax benefit, attendance allowance, pension credit, blue badges for cars for the disabled

· Attended Employment Support Allowance medical assessment

· Carried out the necessary paperwork/letters for Discretionary Housing Payments appeal

· Sorted out problems with housing benefit after bereavement

· Attended tribunals

· Assist to complete various forms - e.g. Blue Badge

· Appeals - with regard to attendance allowance - Personal Independence Payment
