Older Persons Service - available throughout Ceredigion and North Pembrokeshire
The Older Person's service provides a housing related support service to those who live in need in any tenure (irrespective of landlord or individuals can privately owned their property). The only criteria is that the person has to be 50 and over.
The aim of this service is to assist and support individuals to live as independently as possible in the home of their choice. We provide guidance and support with benefits, financial matters, housing issues, accessing other statutory and voluntary services and help with integrating into their local community etc.
The support is tailored to meet individual's needs and it enhances their life by enjoying wellbeing and a good quality existence. To reach positive outcomes we work collaborative with other agencies. We provide support to older adults with mental health needs and learning disabilities, including individuals with memory loss and age related physical needs, such as mobility issues. As well as this staff provide emotional and psychological support The service assists to avoid the need for costly interventions and alleviating burden on statutory services in the future. We are delivering support that ensures not to de-skill individuals or create dependence, but work in an enabling way.
